Please upload clear, valid documents to verify your identity, address, and date of birth. For proof of identity and proof of address, use Aadhaar card issued by UIDAI. For proof of date of birth, you may choose one of the following: ABHA card, voter ID card, matriculation certificate, or birth certificate issued by municipal council.
